SUMMARY:Intrusive by Default? Navigating Privacy by Design in LLM-Supported
  Care Documentation
DESCRIPTION:BritCHI 2026 · Paper Session 4: Digital Health\, Care and Well
being\nKletter\, Koeszegi\nIn an LLM-based documentation support system fo
r residential nursing homes\, audio recordings of care worker–resident i
nteractions are used to generate brief care reports. Drawing on ethnograph
ic observations\, we show that inconsistent privacy practices reflect a pr
ivacy calculus shaped by concerns about workplace surveillance. We examine
the limits of operationalising Privacy by Design in a context where priva
cy concerns are intertwined with distrust toward both technology and the c
are organisation. As a technology probe to explore the technical potential
for privacy protection\, we developed an LLM-based privacy detection pipe
line. While approximately three-quarters of pipeline-identified segments w
ere confirmed as privacy-sensitive\, unreliable speaker diarisation remain
s a major challenge. We argue that care workers should retain autonomy ove
r their privacy preferences rather than relying on predefined technical sa
feguards. Our findings suggest that technical Privacy by Design measures a
lone are insufficient for establishing trustworthy AI documentation suppor
t systems in situated care work.
